What if I don’t have access to the Internet?
DriverFacts also offers fax-in and mail-in requests.  Since the these requests do involve more time and substantial human activity, there is an additional $4.00 manual request fee.  You must use a Non-Internet Request Form (NIR) to request information by fax or mail.

To get the NIR form, print it from www.DriverFacts.com/forms/NIForm.pdf.   Or (if your don't have internet access) call (888) 844-4730, press option 6 at the main greeting, enter your fax number, and a Non-internet Request form will be faxed to you.  (The fax-back system does not accept toll-free numbers).

Once you receive the NIR form, fill it out completely and fax it to the number on the form.  If the form is filled out correctly and the proper signature or valid drug & alcohol release form is received by DriverFacts, your request will be processed within 1-2 business days.  If the form is incomplete or it is not accompanied by the proper signature or release form, your request will be delayed until the problems are corrected.

Does DriverFacts process mail requests?
Yes.  Follow the instructions above and include a self-addressed stamped envelope.   Payment and mailing instructions are located on the form and at the Payment Options screen in the FAQ's.
