| To obtain a Free
D&A + Accidents Safety Report follow these simple steps (This option is available to motor carriers with a valid DOT number) |

| A. | Go to www.DriverFACTS.com.
Click on the 
button. |
| B. | Login with your User Id and Password. (If you don't have an Id and Password, you will have to create one before you can enter the system.) |
| C. | Enter the Social Security Number of the applicant or individual you are searching. Click Search. (If the SSN is found in our system, you will be taken to the next screen. If the SSN is not found in our system, an error message will be displayed and you will be able to submit a Request to Locate a Missing SSN.) |
| D. | The Employment Search Results screen will be displayed. Click the icon for the employer that you are requesting information from. If you need information for multiple employers shown, you will need to return to this step and do it again for the other employers. |
| E. | Click Order Report. |
| F. | Scroll down and click on the button next to Single D&A Safety Report By US Mail. Click Continue button at the bottom of the page. |
| G. | Fill in the information on the form. The form must be filled out completely in order for DriverFACTS to process it. Read the DriverFACTS agreement and click the check box. When you are finished, click the I Accept button at the bottom of the page. |
| H. | A form will print out for you
to sign AND for the applicant to sign (or attach a valid signed release). Mail it to the address
on the form along with a stamped-self addressed envelope.
**DriverFACTS cannot process your request unless you send a stamped-self addressed envelope and fill in the form completely. DriverFACTS cannot guaranty US Mail delivery. **It is your responsibility to ensure that the form prints out completely. Due to the various types of printers on the market, we cannot test every printer. |
